Before you get started with the health inequalities self assessment there are a few things that we feel are good to know. If you have any questions relating to the tool we have created a FAQ section which addresses some of the most commonly asked questions. You can also get in touch with our team if you have any queries.
Create an Account
You can choose to complete this self assessment online or by downloading a printable version. To complete the online version you have to create an account. This will allow you to save your progress and return to continue the self assessment over multiple sittings.
If you already have an account then you should login if you have not already. If you need to create an account it only takes a few moments.
If you would prefer to complete the self assessment by filling in a printable version of the self assessment then you can download this now. The printable version contains all the same questions as the online version.
The downloadable version is made available as a PDF document.

We have prepared two guides to help you throughout completing the self assessment. There are two versions available. The first is a what we call a “Quick Guide” which provides an overview of the assessment and a summary of the guide. We also have the “Full Guide” available to download.
